Home Health

We support patients of all ages with various types of conditions that require home health care such as chronic medical conditions, congestive heart failure, cystic fibrosis and more. Services include wound care, dressing changes, IV therapy administration and catheter care.Nurses perform assessments, measure vital sign and make sure our clients understand how to take their medications or treatments safely. Services range from one-time visits to daily visits.

Case Management

The Serengeti Care case management model allows us to be engaged in your care just like a family member. We offer assistance with insurance claims, hospital and clinic visits, care conferences and the overall plan of care.
